Hemorrhoidectomy (Milligan-Morgan / Ferguson)
Conventional surgical removal of haemorrhoids — the most definitive treatment for advanced grade 3 and grade 4 piles, with the highest long-term success rate.
What is Hemorrhoidectomy (Milligan-Morgan / Ferguson)?
Hemorrhoidectomy is the conventional surgical removal of haemorrhoids and remains the most definitive treatment for advanced grade 3, grade 4, and large mixed haemorrhoids. Two main techniques are used — Milligan-Morgan (open hemorrhoidectomy) and Ferguson (closed hemorrhoidectomy) — depending on the size, location, and type of haemorrhoids. Although recovery is slightly longer than minimally invasive options, hemorrhoidectomy offers the highest long-term success rate and lowest recurrence. As a female proctologist in Hyderabad, Dr. Tejasree Vengala performs hemorrhoidectomy at Lux Hospitals, Hyderabad, with attention to careful technique and structured pain management.
How the Procedure Works
Anaesthesia & Positioning
Surgery is performed under spinal or general anaesthesia. The patient is positioned in lithotomy or prone jackknife position.
Hemorrhoidal Vessel Ligation
Each haemorrhoidal pedicle is carefully identified and ligated with absorbable sutures.
Excision of Haemorrhoids
The haemorrhoidal tissue is excised in an elliptical pattern, taking care to preserve the anal sphincter and adequate skin bridges.
Wound Management
In Milligan-Morgan technique, wounds are left open to heal naturally; in Ferguson technique, wounds are closed with absorbable sutures.
Recovery & Discharge
Patients are discharged in 1–2 days with structured pain management, sitz baths, dietary advice, and clear follow-up plan.
