What is Laparoscopic Common Bile Duct Exploration (LCBDE)?

Laparoscopic Common Bile Duct Exploration (LCBDE) is an advanced keyhole technique to remove stones directly from the common bile duct, usually combined with laparoscopic cholecystectomy in the same sitting. This single-stage approach is particularly useful for patients in whom ERCP is not suitable, has failed, or where it is preferable to clear the duct and remove the gallbladder together. Dr. Tejasree Vengala performs LCBDE at Lux Hospitals, Hyderabad, offering an effective, minimally invasive solution that avoids the need for repeated procedures.

Suitable for patients with bile duct stones (choledocholithiasis) discovered during gallbladder surgery, those in whom ERCP is technically not feasible, or when single-stage clearance of stones and gallbladder removal is preferred.

How the Procedure Works

1

Anaesthesia & Imaging

Surgery is performed under general anaesthesia. Intra-operative cholangiography or ultrasound confirms the location and number of bile duct stones.

2

Laparoscopic Access

Standard laparoscopic ports are placed, similar to a routine keyhole gallbladder surgery.

3

Bile Duct Exploration

The bile duct is approached either through the cystic duct (transcystic) or by a small opening in the bile duct (choledochotomy).

4

Stone Removal

A fine choledochoscope is used to visualise the duct, and stones are removed with baskets or balloons under direct vision.

5

Closure & Gallbladder Removal

The bile duct is closed primarily or over a T-tube, the gallbladder is removed in the same sitting, and patients usually go home within 2–3 days.

Who Needs This Treatment?

  • Single-stage clearance of bile duct stones and gallbladder
  • Avoids the need for separate ERCP procedure
  • Minimally invasive — only small keyhole incisions
  • Direct visualisation of the bile duct using a choledochoscope
  • Faster recovery and shorter overall hospital stay
  • Useful when ERCP is technically not feasible
"

When a bile duct stone shows up unexpectedly during gallbladder surgery, LCBDE allows us to take care of everything in one sitting — no second admission, no second anaesthesia. It is technically demanding, but it is exactly the kind of complete care patients deserve.
