Completion Thyroidectomy
Removal of the remaining thyroid lobe in patients who initially had a hemithyroidectomy and are later diagnosed with cancer — completing the cancer clearance and preparing for radioiodine therapy.
What is Completion Thyroidectomy?
Completion Thyroidectomy is the surgical removal of the remaining thyroid lobe in patients who originally underwent a hemithyroidectomy and were subsequently diagnosed with thyroid cancer on biopsy. By completing the removal of all thyroid tissue, the surgery enables full cancer clearance, allows safe radioiodine therapy, and simplifies long-term follow-up with thyroglobulin monitoring. Dr. Tejasree Vengala performs completion thyroidectomy at Lux Hospitals, Hyderabad, with meticulous re-identification of the recurrent laryngeal nerve and parathyroid glands in previously operated tissue.
How the Procedure Works
Anaesthesia & Planning
Surgery is performed under general anaesthesia. Detailed planning is done based on the original surgery, biopsy report, and any imaging findings.
Re-Opening of the Scar
The original neck incision is re-opened, and tissue planes are carefully redeveloped through scar tissue from the previous operation.
Re-Identification of Structures
The recurrent laryngeal nerve, parathyroid glands and remaining lobe are carefully re-identified, with extra attention given to scar tissue.
Lobe Removal
The remaining thyroid lobe is fully removed along with any residual thyroid tissue, completing the total thyroidectomy.
Closure & Recovery
The wound is closed in layers with absorbable sutures and skin glue. Most patients are discharged in 2 days, with planning for radioiodine therapy when indicated.
Outcomes
Who Needs This Treatment?
- →Completes cancer clearance after initial hemithyroidectomy
- →Allows safe radioiodine ablation when indicated
- →Simplifies long-term cancer surveillance
- →Same well-concealed neck crease scar — no new incision
- →Voice and parathyroid function carefully re-assessed and preserved
- →Standardised long-term follow-up with thyroglobulin testing
Completion thyroidectomy is technically demanding because we are working through scar from the first surgery — but it is also deeply reassuring for patients, because it closes the loop on cancer treatment and opens the door to radioiodine when needed.
