Subtotal / Near-Total Thyroidectomy
Removal of most of the thyroid gland leaving a small remnant — chosen for selected benign conditions and goitres where preserving a small amount of thyroid tissue is preferred.
What is Subtotal / Near-Total Thyroidectomy?
Subtotal or Near-Total Thyroidectomy is the surgical removal of most of the thyroid gland, leaving a small remnant of healthy thyroid tissue behind. It is chosen for selected benign conditions such as large multinodular goitre, certain cases of Graves' disease in select patients, and situations where a small amount of thyroid tissue is preserved to maintain hormone production while controlling the disease. Dr. Tejasree Vengala performs subtotal and near-total thyroidectomy at Lux Hospitals, Hyderabad, with meticulous nerve and parathyroid preservation and excellent cosmetic outcomes.
How the Procedure Works
Anaesthesia & Preparation
Surgery is performed under general anaesthesia. The neck is positioned in slight extension and the area is prepared and draped.
Neck Crease Incision
A small 5 cm incision is placed in a natural neck crease for the best cosmetic outcome.
Bilateral Lobe Mobilisation
Both thyroid lobes are carefully mobilised from surrounding structures, with attention to large neck vessels and strap muscles.
Nerve & Parathyroid Preservation
Both recurrent laryngeal nerves and parathyroid glands are identified and preserved. Most of the gland is removed, leaving a small remnant on each side.
Closure & Recovery
The wound is closed in layers with absorbable sutures and skin glue. Most patients are discharged in 2 days with minimal scarring.
Outcomes
Who Needs This Treatment?
- →Preserves a small amount of functioning thyroid tissue
- →May reduce or avoid the dose of lifelong medication
- →Effective for large benign goitres and select Graves' disease
- →Voice and parathyroid function carefully preserved
- →Small, well-concealed neck crease scar
- →Quick discharge and rapid return to work
Subtotal thyroidectomy is a thoughtful middle path — taking out enough disease to fix the problem, but leaving enough healthy tissue behind to keep the patient as close to natural physiology as possible. Patient-by-patient judgement is everything.
